Effect of Silver Diamine Fluoride and Potassium Iodide Pretreatment on the Microleakage of Resin-based Pit and Fissure Sealants: An in Vitro Study

Aim: To evaluate the effect of silver diamine fluoride and potassium iodide pretreatment on the microleakage of resin-based pit and fissure sealants. Materials and Methods: Forty-five extracted, non-carious human premolars were randomly allocated into three groups (n = 15): Group I (control) received resin-based pit and fissure sealant without pretreatment; Group II received 38% SDF prior to sealant placement; and Group III received 38% SDF followed by KI before sealant application. After sealant placement, all specimens were thermocycled for 500 cycles between 5°C and 55°C. The teeth were coated with nail varnish, immersed in 0.5% basic fuchsin dye for 24 hours, sectioned buccolingually, and evaluated for dye penetration under a stereomicroscope. Microleakage scores were analyzed using the Kruskal–Wallis test followed by Dunn's post hoc test, with the level of significance set at p < 0.05. Results: The control group demonstrated the lowest microleakage scores, while the SDF-pretreated group showed the highest microleakage. The SDF + KI group exhibited lower microleakage than the SDF group but higher values than the control. The differences among the groups were statistically significant (p < 0.05). Conclusion: Pretreatment with SDF influenced the marginal sealing ability of resin-based pit and fissure sealants by increasing microleakage. The application of KI following SDF reduced microleakage compared with SDF alone; however, it did not restore sealing ability to the level of the control group. Further clinical studies are recommended to validate these findings.
